Role Overview
Provide skilled nursing services to patients with complex medical needs in home and community-based settings. This role requires strong clinical judgment, respiratory and airway management experience, and a commitment to patient-centered care.
Key Responsibilities
• Deliver skilled nursing care per physician orders and care plans
• Perform advanced clinical tasks including tracheostomy and oxygen/ventilator care (as applicable)
• Monitor patient condition and respond to changes
• Educate patients and families on care and treatment
• Document services accurately and timely
Requirements
• Active RN license in good standing
• CPR/First Aid certification
• Current TB test
• Valid driver’s license
• Clean background and drug screening
